When catastrophic illness strikes, an individual needs hospital care. Not all patients are ready to return home when their crisis has passed. Some need specialized care and rehabilitation before returning home. PATHWAYS TRANSITIONCARE ® helps patients make a successful transition from the hospital to their homes. Our comprehensive rehabilitation program helps patients regain lost abilities and learn new skills for independent living. The average length of stay for a patient in Pathways is less than three weeks.
