These professionals provide additional support, guidance and assistance to individuals and their families. Care managers assist with navigating the complex health care system, while also helping to assess needs, identify resources and provide additional support.
Geriatric Care Managers assist families in a number of ways and they can tailor their services to address specific client needs. They can assist with multiple medical issues, clarify health care options and help to identify available services and solutions. Care Management Services are not covered by insurance and are privately or self-paid for by the client.
